Shoulders

Improper shoulder alignment (rounded/forward shoulders, protracted or poorly controlled scapulae, forward head posture, or uneven positioning) does not always cause immediate pain, but it alters joint mechanics and muscle balance. Over time—especially when sustained in daily life or loaded during workouts—it can make you stiff, less agile, and leave you feeling older.

By implementing the following Ball Grip Handle exercises, you will achieve the following:

  • Reduce or eliminate shoulder pain
  • Improve shoulder mobility, neutral posture, and proper shoulder alignment
  • Reduce mechanical stress, while unlocking gains
  • Build strength, increase shoulder width, rounding out and widening your shoulders
  • Improve grip strength
  • Improve your physique

Shoulder Exercise 1

Cable Machine Reverse Flys Shoulder Height ("OK Sign" Hand Grip)

  • Adjust the Carriage on both sides of the cable machine to shoulder height.
  • Grip ball in left hand from right carriage and vice versa. Step back one to two steps.
  • With arms in front of your body, elbows slightly bent, and shoulders back and down, pull back in a straight line with the cable machine cord.
  • Squeeze the scapula together the entire exercise
  • Hold in the contracted position of each rep for one or two seconds and slowly return to the starting position.
  • Keep the core engaged by pulling your belly button towards your spine the entire exercise.

Additional variation - lower the carriage 2-3 holes/notches and repeat

Shoulder Exercise 2

Cable Machine Reverse Flys Waist Height (Neutral or Baseball Hand Grip)

  • Adjust the Carriage on both sides of the cable machine to naval or waist height.
  • Reduce the weight by 5-10 lbs from shoulder height Reverse Flys.
  • Grip ball in left hand from right carriage and vice versa. Step back one to two steps.
  • With arms in front of your body, elbows slightly bent, and shoulders back and down, pull back in a straight line with the cable machine cord.
  • Squeeze the scapula together the entire exercise and hold in the contracted position for one or two seconds on each rep.
  • Lower the weight slowly, being mindful of not letting the cable angle change.
  • Keep the core engaged by pulling your belly button towards your spine the entire exercise.

Shoulder Exercise 2

Cable Machine Reverse Flys Feet Height (Neutral or Baseball Hand Grip)

  • Adjust the Carriage on both sides of the cable machine to the lowest height.
  • Reduce weight (5-10 lbs)
  • Grip ball in left hand from right carriage and vice versa. Step back one to two steps.
  • With arms in front of your body, elbows slightly bent, and shoulders back and down, pull back up in a straight line with the cable machine cord.
  • The angle of the cables should not change during the exercise.
  • Squeeze the scapula together the entire exercise and hold on for one or two seconds on each rep.
  • Keep the core engaged by pulling your belly button towards your spine the entire exercise.

Shoulder Exercise 4

Cable Machine Front Raise from Behind (Neutral or Baseball Hand Grip)

  • Lower the carriage to the lowest height possible.
  • Stand facing the opposite carriage, slightly to the left of center. 
  • Reach back slightly and grab the BGH with your right hand using a neutral full palm grip (palm down).
  • With your shoulders back and down raise the BGH in front of your body.
  • The BGH should not go above shoulders, and the angle of the cable should not change during the movement.
  • Pause in the contracted position for one or two seconds and slowly lower the weight to the starting position.
  • Repeat for the left hand, making sure to stand slightly to the right of center.

Shoulder Exercise 5

Cable Machine Front Raise (Neutral or Baseball Hand Grip)

  • Lower the carriage to the lowest height possible.
  • Stand facing the carriage, slightly to the left of center. 
  • Grab the BGH with your right hand using a neutral full palm grip (palm down). 
  • With your shoulders back and down raise the BGH in front of your body.
  • The BGH should not go above shoulders.
  • The cable should be perpendicular to the floor during the entire movement.
  • Pause in the contracted position for one or two seconds and slowly lower the weight to the starting position.
  • Repeat for the left hand, making sure to stand slightly to the right of center.

Shoulder Exercise 6

Cable Machine Bent Over Flys ("OK Sign" Hand Grip)

  • Lower the carriage to the lowest height possible.
  • Grip the BGH in left hand from right carriage and vice versa. The cable goes in between the thumb and the pointy finger.
  • Facing forward step back one to two steps and bend over so that your body is close to parallel to the floor.
  • Make sure to center your body between the carriages.
  • With arms hanging down, elbows slightly bent, and shoulders back and down, pull the BGH's back in a straight line with the cable machine cord.
  • Squeeze the scapula together the entire exercise
  • Hold in the contracted position of each rep for one or two seconds and slowly return to the starting position.
  • Keep the core engaged by pulling your belly button towards your spine the entire exercise.

Common problems from Improper Shoulder Alignment in Daily Life

Prolonged sitting, phone/computer use, driving, one-sided carrying, or general slouching often leads to:

  • Muscle imbalances — Tight/shortened pectorals, upper trapezius, and levator scapulae; lengthened/weak mid- and lower trapezius, rhomboids, serratus anterior, and external rotators. This is frequently described as an upper-crossed pattern.
  • Neck pain, stiffness, and tension headaches — Overworked upper traps and suboccipital muscles, plus altered cervical loading from forward head posture.
  • Upper-back and shoulder aching or stiffness — Reduced thoracic mobility and constant low-level strain on soft tissues.
  • Scapular dyskinesis or mild winging — Poor scapular control or positioning that can feel unstable or “stuck.”
  • Reduced shoulder mobility and a sense of pinching or restricted reach overhead or behind the back.

Not everyone with rounded shoulders develops pain—bodies adapt—but sustained poor positioning plus low activity or repetitive stress increases cumulative risk.

Common problems from Working Out with Improper Alignment

When the same positioning (or poor form that allows the shoulders to roll forward, shrug excessively, or lose scapular control) is loaded with weights, volume, or overhead work, the mechanical stress is magnified. Common resulting issues include:

  • Shoulder impingement (subacromial impingement syndrome) — The space under the acromion narrows, pinching the rotator cuff tendons (especially the supraspinatus) and/or subacromial bursa during arm elevation or certain pressing/raising positions. This is one of the most frequent complaints among lifters with rounded posture or limited thoracic extension.
  • Rotator cuff tendinopathy, tendinitis, or progressive tears — Repetitive compression, overload of the stabilizers, or altered mechanics force the cuff to work in compromised positions. Programs heavy on pressing and delts while neglecting external rotation and scapular stabilizers heighten this risk.
  • Bursitis — Inflammation of the subacromial bursa secondary to impingement or friction.
  • Scapular dyskinesis (worsened under load) — Abnormal scapular movement that further disrupts shoulder rhythm and can contribute to ongoing impingement or feelings of instability.
  • Anterior shoulder stress or mild instability symptoms — Especially with positions that place the arm in abducted/externally rotated “high-five” loading under fatigue or with poor control.
  • Secondary neck, upper-back, or biceps tendon irritation from compensation patterns.
  • Reduced strength, mobility, and performance, plus higher chance of acute tweaks during presses, rows, upright rows, or overhead lifts.

These problems are especially common when daily desk posture carries into the gym, when form breaks down under fatigue, or when training emphasizes anterior muscles without balancing posterior and stabilizer work.

Bottom Line

Improper alignment creates a less favorable environment for the shoulder joint (narrower subacromial space, altered force distribution, and imbalanced muscle demand). In everyday life this tends to produce gradual stiffness, neck/upper-back discomfort, and headaches. Under the higher loads of resistance training it more readily produces impingement, rotator-cuff irritation, and related overuse injuries.

Correcting positioning, improving thoracic mobility, balancing push/pull work, and strengthening the scapular stabilizers and external rotators are the main protective strategies. Persistent or sharp pain warrants assessment by a physical therapist or qualified clinician.
